Please complete all New Hire and Benefit Forms (if applicable) and return to HR at your earliest convenience so that we can ensure you will be set up properly in our system and have your benefit coverage in effect accordingly. The links to access these forms are to the left of this page, under New Employee Orientation.  

Please remember an important time-sensitive requirement to start employment:

  • Complete the I-9 Form on or before your first day (original documents are required for verification, copies are not accepted).

Benefit-Eligible Summary

If your offer letter states that you are benefit-eligible, the effective date of your benefits will begin on the first day of the month following your hire date. Please review the Summary of Employee Benefits and complete as much of the following documents as possible. If there are forms you are unable to complete, we will discuss at your New Hire Orientation.
