Please complete all New Hire and Benefit Forms (if applicable) and return to HR at your earliest convenience so that we can ensure you will be set up properly in our system and have your benefit coverage in effect accordingly. Please complete the Background Check Disclosure Release Form, along with your signed offer letter or contract, and return to Vivian Connor in Human Resources immediately. Forms may be scanned and emailed to firstname.lastname@example.org or faxed to (704) 463-3012.
Please complete and scan these forms back to email@example.com, at your earliest convenience.
IMPORTANT: You will need to bring your I-9 original documents with you on your first day for employment verification. Please be sure to review the last page of the I-9 form for the list of acceptable documents. You will not be allowed to work until you have complied with this request.
Must bring ORIGINAL DOCUMENTS on first day of employment.

If your offer letter states that you are benefit-eligible, the effective date of your benefits will begin on the first day of the month following your hire date. Please review the Summary of Employee Benefits and complete as much of the following documents as possible. If there are forms you are unable to complete, we will discuss at your New Hire Orientation.
